Rebase token-usage onto 026_completed_dependency_ids (linear chain, single head). Also: test_ceo_reject_routes_coordination_task_to_main_pm asserted the old NEEDS_REVISION behavior; the lifecycle fix correctly routes a coordination root to PENDING (Main PM's claim source).
