* Add safe retry policy contracts

* Cap default safe run retry at one attempt

Issue #3543 scopes the first implementation to at most one automatic
same-run retry. Set DEFAULT_SAFE_RUN_RETRY_MAX_ATTEMPTS to 1 so a first
failed retry (attemptCount: 1) suppresses with attempt_limit_reached
instead of being eligible for a second retry, and document that the field
counts retry attempts rather than the initial run. Update the policy test
to assert the one-retry-by-default contract.
