docs(plugins): record DD-series landing in living plan + CHANGELOG

Plan DD3 — sync docs/plans/plugins-implementation.md and CHANGELOG.md
with the DD-series shipped this turn:

  DD1. `od plugin stats` inventory health report
  DD2. `od plugin canon --check <expected-file>` byte-equality fixtures
