WesandCarl 3217003db1 ci: make file-size policy a first-class gate ...

Run one repository-level differential ratchet from pre-push and CI instead
of hiding platform coverage inside lint commands and duplicated path maps.
Keep ratchet Git subprocesses isolated from hook-exported repository state so
the same policy command is safe inside pre-push.
