kazuki 334b6ce146 feat: migrate all plugins to TypeScript with hot reload support
## Major Changes
 Full TypeScript migration (Markdown → TypeScript)
 SessionStart hook auto-activation
 Hot reload support (edit → save → instant reflection)
 Modular package structure with dependencies
