kazuki 334b6ce146 feat: migrate all plugins to TypeScript with hot reload support

## Major Changes
✅ Full TypeScript migration (Markdown → TypeScript)
✅ SessionStart hook auto-activation
✅ Hot reload support (edit → save → instant reflection)
✅ Modular package structure with dependencies
